DIRECT SUPPORT PROFESSIONAL

 

The primary purpose of this position is to provide direct assistance and support for individuals with hygiene, nutrition, housekeeping (maintaining the home/property in a safe and sanitary manner), behavioral management, safety, vocational, health care, finances and other daily activities within the home and/or community.

 

Essential Duties and Responsibilities:

50 % -  Provide needed assistance, and/or support for each individual in whatever degree necessary in regards to behavioral management, nutritional management, recreational activities, community integration, vocational, health care, hygiene, housekeeping, financial management, safety and other daily and/or community activities.

10 % -  Through provided supports, monitor individuals for changes in health, behavior and/or other areas and report observations immediately to management, staff, medical personnel etc. as needed.

10 % -  Document in individuals’ records information relative to the person’s health, behavior, relationships, activities etc. to assist health professionals and those supporting the individual in understanding their past, providing for the present and planning for future needs.

10 % -  House keeping, both interior and exterior including but not limited to: yard maintenance, dishes, laundry, sweeping, mopping, vacuuming, meal preparation, food handling and storage.

5 % -  Attend required training and participate in meetings.

5 % -  Assist in maintaining and promoting a safe environment reporting concerns or issues to management/supervisor.  Conduct/assist safety checks, fire drills, and/or vehicle checks.  Implement proper nutritional support, behavioral intervention techniques and other assistance as trained.

5 % -  Adhere to agency policies, procedures, rules and guidelines and support fulfillment of PI mission statement.

5 % -  Perform other duties as assigned.

 

Minimum Qualifications:

 

*Must be 18 years of age or older;

*It is preferred, not required to possess and maintain valid driver’s license and have a satisfactory driving record and be insurable to drive our company vehicles.

* Must have the ability to pass and maintain First Aid/CPR certification, Oregon Intervention System certification.

* Complete satisfactory criminal history background check, abuse history check and driver’s license check.

* Complete agency required trainings including but not limited to Core Competencies, abuse prevention, blood borne pathogens, medication administration, emergency, Individual Support Plan(s) etc.

* Must be able to read and write English.

*Must be able to lift/transfer 75 lbs.
